Questionnaire on Tick-Borne Disease

Two Palisades residents, both of whom have suffered from tick-borne disease in the last few years, have drawn up a questionnaire about this community problem. They plan to write an article on the subject for an upcoming issue of 10964.
Because the authors ask for a separate questionnaire for each incidence of tick-borne disease, some residents may need more than one copy of the questionnaire. Attached to this article is a pdf file of the questionnaire, which is due by October 31.
